Doctors and Hospitals Team Up to Improve Care in the Region
In 2014, a group of forward-thinking physicians saw an opportunity to bring the medical community together with the purpose of better coordinating medical care and improving the patient experience. These physicians aligned with Gwinnett Medical center to create a coordinated, “clinically integrated” network of physicians to work in partnership with the hospital system. The partnership between physicians and the hospital became even stronger when Gwinnett Medical Center and Northside Hospital merged in 2019, allowing even more physicians to participate in the network.
Sequent Health Physician Partners is the realization of the vision to make quality medical care more convenient and accessible for all patients, provide more personal attention to patients with long-term conditions, and create a greater focus on wellness and staying healthy.

Sequent Boards and Committees
The Sequent Board of Managers is responsible for developing and overseeing implementation of the CIN's mission, vision and strategic plan. The Board provides executive oversight for all CIN operations and final approval for all strategic plans, initiatives, budget, capital expenditure and senior management appointments.
Sequent’s Quality Committee is comprised of physician leaders that provide guidance and oversight for the performance of the network. These physicians help determine the clinical initiatives and the standards by which the network will strive for then work with physicians to achieve those goals. Additionally, the Quality Committee is responsible to:
- Determine and implement clinical strategy
- Increase coordination of care throughout network
- Identify and socialize evidence-based medical practices
- Implement strategies to optimize patient satisfaction
- Develop program of measures that define ongoing value improvement efforts
- Establish quality, cost and utilization standards / targets
- Oversee provider evaluation and intercession process

Northside Hospital is one of Georgia’s leading health care providers with five acute-care hospitals across the Atlanta region, including two in Gwinnett County. Northside Hospital Gwinnett, Northside Hospital Duluth and additional outpatient facilities nearby serve more than 1.35 million patient encounters each year. Northside offers nationally recognized and renowned health care services and expertise that leads the U.S. in newborn deliveries and is among the state’s top providers of cancer care, sports medicine, cardiovascular and surgical services.

Located at the intersection of Pleasant Hill and Howell Ferry Roads, Northside Duluth has 122 beds and expert medical care delivered in a state-of-the-art facility. Services include:
